Health Assessment Coordinator

The Health Assessment Coordinator will manage members and assesses the needs for a targeted population for members with specific chronic conditions identified as requiring interventions to improve quality of care in a cost effective manner. She/he should demonstrate knowledge of basic case management and is responsible for the assessment, and implementation of action plan to meet the individual’s specific needs and goals. She/he is responsible for ensuring that member-selected action plan follow ups are documented monthly, monitored and adjusted, and the reasonable expectations and established outcomes are met. She/he must be able to work as a member of a team to respond appropriately to health alerts and needs of the member within a protocol-based system.
Requirements:
- Bachelor’s degree or currently working towards a Bachelor’s in an allied health discipline; Associates degree with equivalent experience (including EMT, CNA, or Medical Assistant) may be substituted
- 2 years of directly related experience in health care, with demonstrated knowledge of case management issues with specific diseases

- Demonstrated clinical expertise in specialty areas, diseases and conditions

- Professional customer service and calm telephone demeanor
- Proficient PC skills and demonstrate skillful usage of various software, spreadsheets, and medical databases
- Exceptional Administrative and organizational skills
- Ability to take initiative and work independently
- Ability to prioritize and handle multiple tasks
- Attitude and skills that facilitate patient engagement in their health care
- Certification in an allied health care field (i.e. EMT, CNA, Medical Assistant) or LPN preferred
